MANAGING NURSING STAFF AND IMPROVEMENT OF THE QUALITY OF NURSING CARE IN PRIMARY HEALTH CARE AFTER THE INTRODUCTION OF ISO STANDARDS

Management of health care units, including non-public PHC, units should be oriented towards the provision of good quality services. Quality Management System Standards ISO 9001:2001 are intended to help the managers in the provision of an adequate level of services. The objective of the studies was evaluation of the effect of nursing management staff on the improvement of the quality of nursing care in non-public PHC units after the introduction of ISO standards 9001:2001. The studies were conducted by means of a diagnostic survey with the use of a questionnaire form, among 14 members of managing nursing staff of non-public PHC units in the city of Częstochowa. During the period 2002-2005, these units were converted from Independent Public Health Units into Independent Health Units. The studies covered 100% of the staff managing nursing teams in non-public PHC in Częstochowa. Statistical analyses were performed by chi-square test (χ2) of deviations from the uniform distribution, and χ2 test of independence, and binomial test. For 2x2 tables, due to the small study group, Yates’ correction was applied. 5% Type I error was adopted, i.e. p values less than 0.05 were considered as statistically significant. It was noted that the implementation of the Quality Management System Standards ISO 9001:2001 exerted a positive effect on the quality of nursing services in non-public PHC units. The implementation of the ISO system 9001-2001 induced formalization and documentation of the processes realized in Independent Health Units, arrangement and unification of the surveillance over the purchase and use of medical equipment, as well as facilitation of the flow of information. Both the managing nursing staff and the remaining staff of the Independent Health Care Units were engaged in the implementation of the Quality Management System ISO 9001:2001 in non-public PHC units. They participated primarily in the development of the processes and instruction for their workplaces. The staff of Independent Health Units, managed based on the ISO 9001:2001 System, reported that the implementation of ISO 9001:2001 resulted in making the scope of responsibility, duties and authorities more detailed, as well as precise determination of the qualification requirements for the staff at each workplace. The effect and engagement of the managing nursing staff on the improvement of the quality of nursing care in non-public PHC after the implementation of the Quality Management System Standards ISO 9001:2001 was evaluated in positive terms. The staff of Independent Health Care Units are of the opinion that the introduction of the ISO 9001:2001 System allowed more detailed scope of responsibility, duties and authorities, and qualification requirements for the staff at each workplace.
