General Data Protection Regulation (RODO)

 
Pursuant to Art. 13 of the Regulation 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the European Union UE L 119, p. 1) hereinafter referred to as RODO, we hereby inform that:

  1. The Administrator of your personal data is the Witold Chodźko Institute of Rural Health, Jaczewskiego 2, 20-090 Lublin, NIP 7120103781, REGON 000288521.
  2. The Data Protection Officer is Mr. Paweł Wierzba, e-mail: iod@imw.lublin.pl.
  3. The Administrator will process your personal data, especially based on Article 6, para. 1, lit. a) RODO, in order to deliver the newsletter.
  4. Your personal data will be kept for the subscription period.
  5. The provision of personal data is voluntary; however, it is a condition for the delivery of the newsletter.
  6. The recipients of your personal data may be in particular: hosting provider at the Institute, software provider, and e-mail service provider.
  7. Your have the right to access the content of your data and the right to rectify them, data deletion, restriction of processing, the right to data portability, the right to object, the right to withdraw consent to their processing at any time without affecting the lawfulness of processing, which was made on the basis of consent expressed before its withdrawal. Statement on the withdrawal of consent for data processing requires its submission in written form at the seat of the Institute, or to the e-mail address iod@imw.lublin.pl.
  8. You have the right to lodge a complaint with the President of the Personal Data Protection Office when you consider that personal data processing violates the RODO regulations.
  9. In all matters with regard to the processing of personal data please contact the Data Protection Officer appointed the Administrator available at the address: iod@imw.lublin.pl.

I express consent for the processing of my personal data in order for you to send me information concerning journals and novelties. I know that I can withdraw this consent at any time.
